I would just give him the information he is asking for. If the claim he's considering trying to make isn't legitimate, the insurance company will let him know that. Even if the dates he requests are on your letterhead, what you are putting into writing is true and accurate. If he requests SOAP notes...

That's a good way to fold fitted sheets, and the only way I know how, lol. Except I fold both sheets lengthwise and inside out - so that I can unfold the sheet right onto the table with no fumbling, since I am mobile and have to do all the table prep in the presence of the clients. I don't chase down clients. I've found the ones who say "call me" are not very serious about making/keeping an appointment, anyway, but I respond with my "office hours" and hand them a business card or ink pen with my info, and cheerfully say, "whenever you have your calender with you and are ready ...

No, it doesn't cover it. Occasionally they will have a spending allotment that they can choose to spend on massage, but frequently what they get is not nearly enough, and they have to buy their shampoo, toothpaste, etc. out of that, too.

You will have MORE problems with clients being allergic to laundry service chemicals than you would guess. I do not see how the laundry service is the answer to your current problem.
